Iyanla Supported LVG Ltd provides person-centred supported living, outreach and community-based support for adults aged 18 and over.

We support individuals to develop greater confidence, maintain their accommodation, build practical life skills and participate more fully in their communities.

Our service is based on the belief that every person should be treated with dignity, listened to and supported to make meaningful choices about their own life.

Who We Are

We are an independent support provider working with adults who may have mental health needs, learning disabilities, autism, social-care needs or difficulties maintaining independent living.

We provide support within supported-living accommodation and through outreach services delivered in people’s own homes and communities.

We aim to create stable, respectful and enabling environments where people feel safe, valued and able to make progress at their own pace.

Our Approach

Our support is person-centred, strengths-based and outcome-focused.

This means that we begin by identifying what the individual can do, what they want to achieve and what support will help them move forward.

Support plans are developed with the person and are reviewed regularly.

We recognise that progress may be gradual and that people may experience setbacks. Our role is to remain consistent, encouraging and responsive while helping individuals maintain safety and control.

WORKING WITH FAMILIES AND PROFESSIONALS

Families, advocates and professionals can play an important role in a person’s support network.

With the individual’s consent and in accordance with data-protection requirements, we maintain appropriate communication with relevant people involved in their support.

We participate in review meetings, share progress updates, raise concerns promptly and contribute to coordinated risk-management and transition planning.
